PHLEBOTOMY SERVICE

If you have been given a form for a blood test, you can have this taken at the surgery strictly by appointment only.

It is necessary to make an appointment on 01923 845 945 or in person at the reception desk.

It may not be necessary to see a doctor to organise a routine blood test if you have no symptoms.  Please call the reception desk to request a blood form.

In both cases, it is ESSENTIAL that you bring your blood test form with you at your appointment with our phlebotomist otherwise you will be asked to re-book for another day.

Please make sure you arrive at least 5 minutes before your appointment.  If for any reason you cannot keep your appointment please call to cancel.
